Please bear with me on a stupid question. OK - DA recommends a 1/3 cut 2-ply top file folder and I couldn't find this anywhere - does he mean just the plain manila 1/3 cut 2-ply TAB file folder?

I'm pretty sure what he means by "top" file folders are those that open from the top with the tab up there. This is opposed to the side-opening folders with the tabs on the side that are often used in shelf filing systems.
So, yes, what you are looking at is what he means.

Yes - I'm sure that's what he means. The 2 ply top refers to the thickness. Its more durable (and a little more expensive) and the tops wont curl over like they would on the single ply.
