I completely agree with Katherine. GTD is a way of managing commitments, not an elixir to make them disappear. It sounds to me like you simply have too much to do, but haven't reached the point...
I use a pretty basic list of contexts...just as many as I need...

I check my e-mail on my phone all the time. Becuase my phone is my PDA I can enter next actions and calendared items on the spot if I have a few minutes while waiting for an appointment. It works...
