The planner needs to be loose leaf. Only day specific things go on the calendar portion. None of your action lists by context are written in any specific day unless the action can only be done on that day.

The action lists should be sorted by context and kept separate. There is a good overview on setting up a paper planner in the files here.

I'd go with one page per context myself. At least that is how I did it for the small amount of time I was using paper.
