I have the same Tools (Exchange/iPhone). Here is my simple GTD setup (7 different lists) :
1 - Next Actions
2 - Tickler
3 - Agendas
4 - Waiting For
5 - Someday Maybe
6 - Projects
7 - Perspective
I don't use categories anymore for the contexts in Outlook but I tag all my Next Actions with the context (e.g. @Calls Philip about GTD Setup number XXX).
You can search in Reminders keywords/contexts to pick up the next stuff to do.
I recently split "Agendas" and "Waiting For" into lists after reading this post :

"Perspective" stands for higher horizons with 4 "tasks" (one for each horizon from 20,000 to 50,000 level) but I'm looking for a better way to handle it (perhaps Mind Map).
